Would like to know if anyone else has had an intermittent condition where the transmission does not actually switch from R to D, or D to R, even though the vehicle indicates so?
This has been chronic issue for me. I have reported two other instances to the dealer, and it is documented. Each time, I was told a software update would be installed.
Tonight however, it caused an accident. As I was leaving a parking spot (on an incline) I put the car in Drive, heard the Reverse 'beep' stop, applied the hill assist function, and pushed the accelerator. The car lunged backward and slammed into the car behind me.
I am taking this quite seriously. Instead of a car, it could have easily been a pedestrian behind me.
Can anyone advise--beyond the dealership-- where I should report this safety hazard at Toyota?
